Improving Coordination, Communication, and the Consumer Experience

The first meeting of the Long Term Care Planning Project will discuss the challenges faced when coordinating and communicating long term care services between agencies and with consumers. We encourage all attendees to come prepared to discuss actions that can help to improve coordination, communication and the consumer experience.
